Confirm caregiver (proxy) access prior to viewing a loved one's record

When you request proxy access to a minor or an adult sends you an invitation to access their records, you get a text or email notification letting you know.

Confirm and accept caregiver (proxy) access

  1. Click the link in the text message or the Log in to MyChart button in the email to view and accept your access.

  2. Click the dropdown arrow to View the Proxy Access Disclaimer.

  3. Enter your loved one’s date of birth and click the checkbox to accept the Proxy Access Disclaimer, then click Confirm.

  4. Click Done to continue to your account.

  5. If you enter an incorrect date of birth three times, proxy access will be denied.

Decline caregiver (proxy) access

  1. If you do not recognize the patient, or you do not want access, click on Decline access.
  2. Enter the reason for declining access, then click Decline.
