Find COVID-19 digital records via QR codes and health wallets

As you start to go back to work, travel, or attend events such as a concert or sporting event, you might be asked if you've been vaccinated for COVID-19 or if you have recent test results. You can share your information quickly, easily, and securely right from MyChart or a separate health wallet.

Does my digital record show the COVID-19 booster too?

If you have had a COVID-19 booster, the digital record displays if the CDC has recommended the booster for you. At this time it appears on your digital record only if you are 50 or older and are at risk for severe COVID-19.

Can I access a digital record of family and friends?

If you have caregiver or proxy access to a child or an adult patient, you can navigate to their account after you log in and download their QR code, print a PDF, or save their QR code to a Health Wallet as described below. You cannot view the digital record for proxies using the pre-login function.
