The ARC Senior Health Connect program
ARC and agilon health partnered in 2018 to provide patients with Medicare Advantage plans with the same kind of connected and coordinated care we already provided to patients with traditional Medicare. Through the Senior Health Connect program, ARC patients are covered by Medicare Advantage plans from Aetna, Blue Cross, Humana, and UnitedHealthcare.
Like with traditional Medicare patients, benefits include access to Medicare wellness visits, support from a patient outreach team to ensure screenings are up to date, a nursing team to monitor the health of patients with chronic illness, and care coordination with specialty physicians and after hospitalization.
Medicare Annual Enrollment Period (AEP) is around the corner
