June 1, 2010 - ARC began its disease management outreach program in June 2010. The purpose of this program is to contact patients who meet certain criteria and attempt to schedule them for follow-up visits with their PCP.
We piloted the program with a list of patients with a diabetes diagnosis who had not been in to see their ARC doctor within the past year. You can find information about the new outreach program on CMSweb. We will update the list of ongoing outreach projects on CMSWeb and include the letters, scripts, and special phone numbers being used. We realize that some patients will contact their clinic directly asking about a letter s/he received from ARC. This list of outreach projects will keep you up-to-date on what has been sent to our patients.
