• New patients will need to complete the forms below prior to be seeing by the Physical Therapist. Please complete these forms at home and bring them with you to your first appointment. Please arrive 15 minutes prior to your appointment so we can process this paperwork and prepare your chart.
• Arrival time – please plan to arrive 5-10 minutes prior to your appointment time so that we may begin on time.
• No show – if you can not make it to your appointment, please call in advance so we can reschedule you and give your appointment slot to another patient. After two no-shows in a row, you will be referred back to your referring doctor for re-evaluation.
• Cancellations – if you can not make it to your appointment, please call in advance so we can reschedule you and give your appointment slot to another patient. After three cancellations in a row, you will be referred back to your referring doctor for re-evaluation.
